Initial Consultation
60- minute video chat. We will take time to discover your ideal vision of best self , explore long and short term goals, narrow on potential focal areas and develop initial action steps

Requires completion of initial consultation
Package includes a total of 4 hours of individual coaching used as a “punch pass.” After each completed follow up session the total time is deducted from initial 4-hour balance. Expires one year after purchase

Functional Health Consultation
Initial Consultation
Includes 60-90 minute virtual or telephone meeting after completing intake assessment forms.
Includes but not limited to medical history review, detailed discussion surrounding environment, diet and lifestyle, review of previous lab work, educating on functional medicine practice and development of action plans.

Follow-Up Visits
Follow up visit are 30-45 minute virtual or telephone meeting after initial consultation has been completed.
Visit includes but not limited to review of any new labs, discuss progress, follow up on treatment plans and make changes as needed.
